New Pet Boarding Questionnaire

Thank you for your boarding request. Please answer the following questions for each pet boarding with us. This information will be saved in our computer system and can be quickly adjusted for future stays Please allow 10 minutes at drop-off time to go over the information/instructions you provided and to sign the Boarding Release Form.

Pets can be picked up Mon-Sat (8-5 pm),Sunday (11-5 pm). Pick up between 3-5 pm if bath is requested.

BOARDING REQUIREMENTS: Proof of up-to-date vaccines ( Rabies, Dapp, Bordetella, Influenza)and negative fecal

Being away from home can be a stressful experience for some pets. I understand that pets can become ill sometimes while boarding and I give permission for treatment and assume payment. Boarding Release Form needs to be signed at time of drop off. I certify that the information on this form is accurate and that I am the legal owner of the named pets and agree to the terms above.
